USA Wrestling's Medical Information & Waiver Form Click Here to view and Print a Copy of USA Wrestling’s Medical Information & Waiver Form. These forms must be completed and Faxed to 800-314-9374 before participation in any wrestling class. Forms can also be submitted to wrestling staff before any session.
USA Wrestling's Notice of Injury Form & Sports Accident Insurance Claim Filing Instructions Click Here to view and Print a Copy of USA Wrestling’s Notice of Injury Form & Sports Accident Claim Filing Instructions. To receive insurance benefits due to an injury sustained while at a sactioned USA Wrestling Tournament or while training at a Regional USA Wrestling Training Site these instruction must be followed.
USA Wrestling's Sports Accident & Liability Insurance Summary Click Here to view and Print a Copy of USA Wrestling’s Sports Accident & Liability Insurance Summary for USA Wrestling Membership. This will familiarize you with policies and procedures involving liability insurance issues connected with injuries occuring while participating in any of the wrestling programs.

To Change Your Club Affilation with USA Wrestling. If you currently have a USA Wrestling Card affilated with another club please change it to the "Lee Kemp Cooler Wrestling Club" by instructing Team Georgia to change your club affiliation their "Contact Us" form.
